Minimum Qualifications and Requirements:

  • High School Diploma
  • Remain free of criminal convictions and drug and alcohol related charges or citations on driver's license. Valid Maryland driver's license with no more than three points currently assessed against you.  Must be able to drive for this position.
  • Physical examination with negative TB, Hepatitis B and communicable diseases screening within 10 days of being hired. Employee must be free from communicable diseases as a condition of continued employment.
  • Trained in (and remain current) in the following, but not limited to, Medication Administration, Fundamental Human Rights, First Aid, C.P.R., Signs of Illness, Signs of Mental Illness, Individual Planning Process, Behavior Management, and Positive Behavior Supports within 90 days of employment.
  • Trained in all areas of individual consumers' special needs before working alone with that Individual.
  • Ability to work flexible hours including overnight coverage as designated by your position.
  • Possess good daily living skills such as food preparation, housecleaning skills, money management skills, etc.
  • Present a positive role model for individuals and other staff.
  • Possess functional reading and writing skills necessary to complete the duties for the Substitute Staff position.

Physical requirements:  While performing the duties of this job, the employee is regularly required to talk, hear, stand, walk, sit, bend, lift, and reach with hands and arms.  The employee will frequently lift and transfer adults with and without equipment and must be able to lift at least 50 lbs.  The employee must maintain skills necessary to safely and efficiently operate various types of equipment and assistive devices such as:  Mechanical lifts, electronic scales, shower and bath equipment, and adjustable beds and chairs.

  • Knowledge of human service industry, including specifics about the field of developmental disabilities
  • Ability to respect and advocate for people's fundamental right's
  • Ability to work effectively with people (staff and consumers), some of whom may require higher levels of patience and diplomacy.  You will receive training but nothing is better than patience and creative problem solving. 
  • Understand the importance of confidentiality and be able to maintain it.
  • Ability to communicate effectively verbally as well as in writing
  • Ability to work with both individuals and on a team
  • General computer knowledge (Microsoft Word, email, attachments) 
  • Washing individual's laundry, individuals should assist if possible, do not combine individual's laundry
  • Shopping for individual's needs, if directed and turning in receipts


Outcomes and Supporting Job Duties


Outcome 1:  Provide outstanding support and services for people

  • Administer and properly document all medications and treatments as ordered. In the event that you are not certified to administer medication, ensure that certified staff or supervisor is notified in a timely manner so that other arrangements can be made. Review all Medication Administration Records at every shift change and immediately identify and follow-up on all medication errors and up-stream this information to the nurse/program team/house manager immediately.
  • Maintain a count of all medications and ensure that all medications are being re-ordered in a timely manner. Ordering medication, supplements, and supplies 5 to 7 days ahead as directed by the house manager.
  • Promptly and properly report all individual illness or injuries to the nurse and administration and make arrangements for the individual to see the appropriate health care practitioner.
  • Promptly and properly alert the administration of any safety hazards in the individual's environment. Take any immediate action within your capabilities to limit safety hazards. Completing facility request for house repairs. Do not assume someone else is reporting.
  • Ensure that living environment remains clean and free of safety hazards.
  • Periodically check on the individual at least hourly or as determined in the IP to ensure that they are safe within their environment while sleeping.
  • Properly implement all Behavior Plans (B.P.) and their emergency protocols to prevent injury to the individual as well as others in the environment. Properly document all activity. All staff who work in the house should know the information
  • Properly implement and document all emergency protocols, i.e. seizure management, First Aid, CPR, behavioral incidents as needed. Ensure that all supporting documentation is submitted to the office at the appropriate time, weekly, monthly, as needed.
  • Must be a CMT and able to administer medication:
  • Must complete 350 hours of work and be in good standings before being registered.
  • Substitute Staff: Can pay for the training at his/her own expense and will be reimbursed after completing 350 hours of work and must be in good standing.
  • Substitute Status: in order for you to remain as an employee (substitute status) with Langton Green, you would need to complete one (1) full shift (6 hours or more) within a month's (30 days) time. If you should fall out of compliance you would be immediately removed from employment and would have the ability to reapply after 3 months (90 days)
  • Knowledge of care plan, initial nursing assessment, 45-day nursing assessment, and certifying in Carematic, follow-up with request from nurse
  • Coordinate the monthly fire drills- once a month with House Manager. (for a shift house that is 4 a month)
  • Write daily diaries- all staff
  • Assist individuals with the finance book/bank account and budgeting- all staff
  • Ensure repair for durable medical equipment is reported to the appropriate person
  • Must meet with the nurse every 6 months

Outcome # 2: Ensure that all individual's chosen daily routines (as identified in their IP) are adhered to:

  • Prepare for work or the day's activities by adhering to the individuals chosen daily routine as noted in the IP.
  • Ensure that the wants, needs, desires and outcomes outlined in the Individual's Plan are adhered to or actively pursued and documented.
  • Ensure that individuals receive well balanced meals as outlined in the menu planning book and IP.
  • Ensure that the individual has necessary supplies for the day which include: medication, a change of clothing, menstruation supplies, incontinence briefs, eye glasses, hearing aids, and any other adaptive devices or supplies and necessary monies for the day.
  • Ensure that each individual completes necessary hygiene activities including: showering, oral hygiene, nail care, toileting activities, health care related needs, etc. with the amount of assistance needed by the individual. Promptly report any failure by staff to properly maintain an individual's hygiene.
  • Ensure that the individuals are dressed appropriately in regards to weather, activity, color coordination, pressed, clean, mended, worn correctly i.e.: tucked in, right side out, shoes on correct feet with laces tied, buttoned, zipped and snapped.
  • Monthly Progress Notes- all staff should be doing

Outcome # 3: Ensure that all individuals are actively engaged in preferred activities and participate fully in the life of the community:

  • Ensure that individual goes on outings and activities of their choice and document their choices. Develop activity schedules that reflect individual preferences and insure that other staff are made aware of the schedule and implement it.
  • Encourage all individuals to experience new activities.
  • Involve the individuals in community/recreational activities

Outcome # 4:  Ensure that all individual's rights and privacy are respected:

  • Ensure that the individual has access to family, friends, and any other interested parties by phone or any other means the individual requires.
  • Ensure that the individual has privacy when using the phone or having visitors.
  • Ensure that the individual has access to the phone.
  • Become familiar with the individuals preferred social network and assist the individual to interact with people and/or organizations of his/her choice.
  • Immediately report both verbally and in writing any possible violations of an individual's rights.
  • Respecting and knowledge of individuals rights, and choice
